Enrolment procedure

The enrolment procedure is important to us. It is our contract with you: our promise to provide a quality program for your child and to ensure that your child is covered by our insurance. It is also your promise to pay us for the service that we provide. More importantly, the enrolment procedure provides us with information about your child and your family, which can help your child to settle into Jabiru Kids.

Your child cannot attend a program at Jabiru Kids until the enrolment forms have been fully completed.

To enrol at Jabiru Kids, you will need to complete two forms, which are available via the links below or in hard copy from your Jabiru Kids centre:

Please complete these forms and return them to your Jabiru Kids centre at least one week before you want your child to commence.

You may also need to complete:

  • Centrelink Forms to apply for Child Care Benefit
  • A Parent Statement Form to notify us of any other children in care

These forms are essential if you wish to receive Childcare Benefit (a government contribution towards the costs of care).

Shared custody
Where families have shared custody of a child, and both parents need childcare, both parents will need to complete separate enrolment and booking forms. Each parent will also need to contact Centrelink to make independent arrangements for Child Care Benefit.
